NC Innovations

Community Navigator -promote self-determination, support the individual in making life choices, provide advocacy and identify opportunities to become a part of their community.

Community Networking Individual/Group -provide individualized day activities that support the waiver beneficiary’s definition of a meaningful day in an integrated community setting, with persons who are not disabled.

Supported Employment Services -provide assistance with choosing, acquiring, and maintaining a job.

Day Supports Individual/Group-facility -based service that provides assistance to the individual developing skills, retention, or improvement in socialization and daily living skills.

Psychosocial/PSR Rehabilitation Services

PSR is a rehabilitative service in a licensed facility designed to assist adults (age 18 and older) with psychiatric disabilities to restore their ability to live successfully in the community. Psychiatric Rehabilitation (PR) focuses on restoration of skills which allow the beneficiary to live independently, manage their illness and reestablish their community roles. PR’s therapeutic goal centers on improving and fostering sustainability within the life domains of emotional, social, safety, housing, medical and health, educational, vocational and legal so that the skills can be practiced within the community.

Positive Relationships

Social isolation has been known as a key trigger for mental illness, while supportive relationships with friends, family and neighbors are beneficial to the mental health of individuals and the population. Volunteering are also known to boost wellbeing.

Referral for Residential Services

We provide NC Innovations Residential Services (Level 1-4) through our sister agency Positive Connection Care, Inc. (PCC).

We provide Residential Supports, Alternative Family Living (AFL) services, Respite, Supportive living, Community Networking and Community Navigator.

About Us

Creative Management Source, Inc. (formerly Case Management Care, Inc.) has been in operations since March 2005. We provided Case Management Services until 2013. Creative Management Source, Inc. is MH/IDD Intellectual Developmental Disabilities agency and we serve adults who have serious and persistent mental illness, or dual diagnosed and those who are Intellectual and Developmental Disabilities.
